Index does not match the table. Delete the index file and re-create the index.
Index file "m:\ld\sysuser\firm.cdx" tag "User_code" is corrupted. Please rebuild it.
firm.cdx is corrupt
replace it from backup
reindex
Note: never replace complete LD folder
